Human error, not equipment failure, may be responsible for the roughly 100 tons of highly radioactive water released from a storage tank in February, the operator of the crippled Fukushima No. 1 power plant said.
When Tokyo Electric Power Co. first disclosed the incident Feb. 20, it said a faulty valve may have allowed water into the tank, which was already nearly full.
But the utility later said human error may have been to blame.
"We have to check the situation," a Tepco spokesman told reporters Feb. 21. (Kyodo)
